Medical Eligibility

All NYU varsity student-athletes are required to provide the athletic training staff with pertinent medical information prior to their participation in a varsity athletics program. The NYU athletic training department uses Athletic Trainer Systems (ATS) for online record keeping and injury documentation.

First Year Student-Athletes:
  1. Create a profile with ATS by following these ATS instructions for First-Year Student-Athletes.
  2. Once you have created a profile, follow the instructions to fill out ALL of the required forms listed.

  3. Once your profile has been created, all forms have been filled out, and the necessary forms uploaded, you are required to print out the physical form (PDF) posted and schedule a physical with your family medicine/primary care physician that is familiar with your history. Your physical should be an in-office visit with your primary care doctor.

  4. After the physical has been completed and your physical form is uploaded into the E-Files section of ATS, you will be asked to schedule a time to meet with a sports medicine staff member in order to review your paperwork and to complete your baseline concussion testing. This appointment will take place once you arrive on campus. More information to follow.

  5. Please have the physical form uploaded to ATS and all the medical paperwork completed no later than August 1, 2022. Failure to meet this deadline may cause a delay in your participation.

Returning Student-Athletes:
  1. By using your Athlete ID and the password you previously created, log into ATS and update your profile following these ATS instructions for returning student-athletes.
  2. If you have had any major medical conditions in the last 12 months, please contact your team athletic trainer by phone or email to discuss necessary paperwork for clearance.

  3. After your ATS profile has been updated and your medical forms have been completed, you will be required to schedule an appointment with a member of the athletic training staff in order to complete your medical update. No walk-ins will be allowed. A link to the appointment scheduling page will be provided at a later date.

  4. Please have all the medical paperwork completed no later than August 1, 2022. Failure to meet this deadline may cause a delay in your participation.
