First Year Student Athletes Header
  1. Copy link: nyu2.atsusers.com into your internet browser. 
    1. You must use Firefox or Google Chrome as Internet Explorer will not work correctly. 
  2. Please fill in as follows: 
    1. N#: new 
    2. Password: new 
    3. Database: atsnyu (leave as is) 
  3. Fill out all highlighted (yellow) sections of the General Information section. Click "save athlete information" once completed. 
    1. Please use your NYU email only (if you do not have it yet, type in an email that is active and go back and change when the NYU email becomes available) 
    2. When asked for an Athlete ID, please enter your "N number" located on the back of your NYU ID card. Your N# starts with "N" and is followed by 8 numbers. 
    3. Create a password of your choosing to remember. 
    4. Once completed, you will receive email confirmation with your login information. Please keep this for your records, as this will be your daily login information when entering the athletic training room, and when making changes to your medical information. 
  4. At the top of your screen you will see: General, Medical History, Insurance, and Contacts. Please click on each tab and fill out all of the appropriate information. Please save each page to make sure you don't lose your changes made. 
  5. Again, remember to SAVE all your changes and keep your N# and password readily accessible for login. 
  6. Once you have created a profile, follow the instructions to fill out ALL of the required forms listed. In the tab labeled Forms, you must complete the following: 
    • FIRST YEAR Medical History 
    • Student-athlete Consent form 
    • Concussion Agreement form 
  7. In the tab labeled eFiles, you are required to print out the pre-participation physical exam (PDF) and the necessary forms, and schedule a physical with your family medicine/primary care physician(s) that is familiar with your history. Please do not use Urgent Care or Minute Clinic. Your primary care physician should complete the 4-page physical form.  No other form will be accepted. First-year international student athletes will have their physical completed with the NYU team physician upon arrival on campus. Upload your physical form into the E-Files tab of ATS
  8. In the eFiles tab, you will see multiple attachments that need to be downloaded and read thoroughly. 
    • Please upload Sickle Cell Solubility Testing results. After reviewing the NCAA Sickle Cell Trait – A Fact Sheet for Student-Athletes, student-athletes must provide their sickle cell trait status (Sickle Cell Solubility Test Results). The NCAA requires confirmation of sickle cell trait status for all incoming student-athletes prior to participation in intercollegiate athletics.
    • If you have been diagnosed with ADD-ADHD, you and your physician must complete the "NCAA Medical Exception Reporting" form and upload the required supporting documentation listed on the form. 
    • If at the time of signage you are under the age of 18, your parents will need to cosign all of the above mentioned forms and they will also need to complete the "Authorization for Treatment of a Minor" form located in this section. Upload this form to ATS
  9. Once you have completed all the appropriate paperwork, you will be asked to schedule a time to meet with a sports medicine staff member in order to review your paperwork and to complete your baseline concussion testing. 
  10. Please have your profile created, the physical form uploaded to ATS, and all the medical paperwork completed by the deadline stated in the email. Failure to meet this deadline may cause a delay in your participation.
